[MDO] Incident Reports with Parent Signature

Raised Sept 25. MDO/childcare programs need documented, parent-acknowledged incident reports (a fall, a bite, an injury) for liability reasons, distinct from general announcements or messaging. Scope: a structured incident report form (what happened, when, who was involved, staff notes), routed to the parent for e-signature/acknowledgment, with a record kept on the child's profile.
